Passwords not working?

I'm having trouble logging into my smartftp. It keeps saying login incorrect, please specify password, but it's the same username and password i've always used. I know I need to get my login info from my Host, but i can't remember who that is or where I put my paperwork that would say who it is. Where would I be able to find out who my host is. Or does anyone know why all of a sudden my login information is no longer working? Any and all help and suggestions will be greatly appreciated.
